Drug hypersensitivity reactions (DHR) have potential for significant morbidity and mortality, without clear treatment severe reactions. Standard of care is to immediately stop culprit drug(s) never rechallenge. However, the drug and/or whether patient would develop a form DHR unknown. Even if reaction severe, may no alternative their underlying disease. Analysis bulk transcriptional profiling skin specimens suggested activation JAK/STAT pathways in mild (morbilliform eruption) (Stevens-Johnson Syndrome/toxic epidermal necrolysis Reaction with Eosinophilia Systemic Symptoms) DHR, pattern consistent both IFNg IL2Rgc signaling. Immunohistochemical staining activated forms protein confirmed increased expression JAK1, 2 3, STAT1 all three DHR. Two cancer patients developed concerning risk progression disease attributed anti-cancer therapy. Cancer was stopped but neither had an available. Both were successfully prophylaxed non-selective JAK inhibitor, ruxolitinib, prior reinitiating suspected drug. In cases, received therapy reacting.
